What is an Intermediate Care Facility (ICF/IID)?

Intermediate Care Facility for Individuals with Intellectual Disabilities (ICF/IID) is a Medicaid-certified facility, to serve individuals in a residential home setting with 24-hour staff. Clients are evaluated and an individualized treatment plan is developed to assist each individual to function at their greatest ability.

Resident Qualifications:

  • The potential resident must have a documented diagnosis of an intellectual disability prior to age 18.
  • Provide a psychological evaluation including an IQ score completed by a licensed psychologist within the past year.
  • Complete the Pre-admission form and application form.
  • The resident must me have the ability to benefit from the active treatment provided in the 24-hour supervised homelike setting.
  • Resident must be Medicaid eligible, preferably approved for Medicaid services.
  • Our residents must be at least 18 years of age or older.

What is the assessment process like?

An assessment is required for admission and can be arranged at the convenience of the family. The assessment process includes completing a pre-admission form, completing an application, meeting with our interdisciplinary team and our staff, as well as touring the facility.

What are Individualized Support Plans?

An Individualized Support Plan is based on a comprehensive assessment by our interdisciplinary team that allows us to uncover each resident’s unique care needs, strengths, goals, and interests. Each plan helps us to ensure our staff is meeting the care needs of our residents, and that each resident is meeting their goals, learning, growing, and thriving. We will provide active treatment in the least restrictive setting that includes all needed services for individuals with intellectual disabilities who require various health care services, supervision, education, and recreational activities on a daily basis.

How often can we visit?

Family and friends are encouraged to visit frequently! We will involve family and friends as much and as often as possible to promote community involvement, civic mindedness, a strong family culture, and a positive purposeful life for our residents and their families.
