What is an Intermediate Care Facility (ICF/IID)?
An Intermediate Care Facility for Individuals with Intellectual Disabilities (ICF/IID) is a Medicaid-certified home where individuals can live in a safe, supportive, and nurturing environment. Our 24-hour professional staff works closely with each resident to create a personalized treatment plan that empowers them to achieve their fullest potential.

Resident Qualifications
To join the Parker Place family, potential residents must meet the following criteria:
- A documented diagnosis of an intellectual disability before age 18.
- A psychological evaluation, including an IQ score, completed within the last year by a licensed psychologist.
- Completion of a pre-admission and application form.
- Medicaid eligibility, preferably with approved Medicaid services.
- Be at least 18 years old and able to benefit from the active treatment we provide in our homelike, 24-hour supervised setting.
What is the Assessment Process?
We make the assessment process as smooth and family-friendly as possible. Here’s how it works:
- Complete Forms: Submit the pre-admission and application forms.
- Meet Our Team: Schedule a meeting with our interdisciplinary team to discuss care needs and goals.
- Tour Our Facility: Explore our homes to see firsthand the welcoming environment we offer.
We’ll work closely with you to ensure the process is convenient and comfortable for everyone involved.
What Are Individualized Support Plans?
At Parker Place, every resident receives an Individualized Support Plan (ISP) tailored to their unique strengths, needs, and goals. These plans:
- Are developed by our interdisciplinary team through a comprehensive assessment.
- Include personalized care, active treatment, educational opportunities, health care services, and recreational activities.
- Focus on fostering growth, independence, and happiness in the least restrictive setting.
